Job summary

Oakwood Gardens Care Center in Fresno, CA is seeking a licensed Physical Therapist for a full-time position at a skilled nursing and rehabilitation facility.

In this role you will evaluate residents, develop treatment plans, provide direct PT, document progress, supervise PTAs, and collaborate with an interdisciplinary team to support discharge planning and caregiver education.

• Document evaluations, progress notes, and treatment records accurately and timely
• Collaborate with an interdisciplinary team to ensure comprehensive resident care
• Supervise and support Physical Therapy Assistants (PTAs)
• Participate in discharge planning and caregiver education to ensure continuity of care
• Maintain compliance with state, federal, and facility regulations

What We’re Looking For:

• Licensed Physical Therapist (PT) in the State of California
• Strong clinical skills and dedication to resident-centered care
• Ability to work independently as well as part of a team
• Excellent communication and organizational skills
• Skilled nursing or post-acute rehab experience preferred, but not required
