Qualifications

• Active and unrestricted Physical Therapist (PT) license in the applicable state.
• Strong clinical assessment, treatment planning, and documentation skills.
• Effective communication with patients, families, and care teams.
• Successfully pass background, TB screening, and drug screening as part of employment.

Responsibilities

• Evaluate patients’ mobility, strength, balance, range of motion and overall functional abilities.
• Develop and implement individualized treatment plans based on patient needs and goals.
• Provide therapeutic exercises, mobility training, gait training and other PT interventions.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and caregivers on exercises, mobility techniques, equipment use, and recovery strategies.
• Collaborate with physicians, nurses and other rehab professionals to support goals.
• Maintain accurate documentation of evaluations, treatment sessions, progress and outcomes.
• Support discharge planning and follow-up therapy recommendations.
• Adhere to clinical, safety, regulatory and infection-control standards.
