You will evaluate patients, develop treatment plans, and provide interventions to improve mobility and independence, collaborating with an interdisciplinary rehab team and ensuring quality documentation and patient-centered care.

Qualifications

• Active and unrestricted Physical Therapist (PT) license in the applicable state of practice.
• Knowledge of physical therapy principles and patient-centered care.
• Strong clinical assessment, treatment planning, and documentation skills.
• Effective communication and interpersonal skills with patients and care teams.
• Ability to work independently while collaborating within a rehabilitation setting.
• Ability to meet the physical demands of the role.
• Successful completion of background check and pre-employment screenings.

Responsibilities

• Evaluate patients’ mobility, strength, balance, range of motion, and functional abilities.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, mobility and gait training.
• Monitor progress and adjust treatment plans.
• Educate patients and caregivers on exercises and strategies.
• Collaborate with physicians, nurses, and rehabilitation team members.
• Maintain accurate documentation of evaluations, sessions, progress, and outcomes.
• Support discharge planning and recommendations.
• Follow safety, regulatory, and infection-control standards.
• Promote a safe, patient-centered rehabilitation environment.
• Other duties as assigned.
