Continuum Therapy Partners is seeking a Physical Therapist to join our team at Meadow View Health Services in Sheboygan, WI. The role requires a Doctor of Physical Therapy (DPT) and an active state license, with NPTE passage. Prior experience of about one year is preferred.

You will evaluate and implement individualized treatment plans in coordination with other disciplines. The position offers a full-time daytime schedule with comprehensive benefits and licensure support.

Physical Therapist Requirements & Responsibilities
