Continuum Therapy Partners seeks a Physical Therapist for Middletown Health & Rehab Center, Middletown, OH. You will assess patients, develop and implement care plans, and collaborate with other specialists to optimize outcomes.

This full-time daytime role offers a comprehensive benefits package and licensure reimbursement, with a sign-on bonus of $5k. Prior PT experience is preferred, and documentation of progress is essential.

Qualifications

• Active state Physical Therapy license and NPTE must be current.
• 1 year of PT experience preferred.
• Evaluates, develops and implements individualized treatment plans with other disciplines.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required

• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)

• 1 year of previous experience as a Physical Therapist preferred

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines

• Documents patient progress and adjusts plans of care based on clinical outcomes

• Educates patients and families on home exercise programs and injury prevention
