Job summary

Gateway Post Acute in Porterville, CA is seeking a compassionate Physical Therapist to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and guide mobility and independence.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current California Physical Therapist license in good standing.
  • Current CPR/BLS certification.
  • Previous skilled nursing or long-term care experience preferred.
  • Strong clinical assessment, treatment planning, and documentation skills.
  • Excellent communication, organization, and interpersonal skills.

Responsibilities

  • Evaluate residents’ mobility, strength, balance, endurance, and functional abilities.
  • Develop and implement individualized treatment plans based on assessments, physician orders, and resident goals.
  • Provide therapeutic exercises, gait training, balance activities, and other appropriate interventions.
  • Help residents improve transfers, mobility, and safe use of assistive devices.
  • Monitor progress and adjust treatment plans to support meaningful outcomes.
  • Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention.
  • Maintain accurate and timely evaluations, treatment notes, progress reports, and discharge summaries.
  • Collaborate with nursing, occupational therapy, speech therapy, and other interdisciplinary team members.
  • Participate in care planning and discharge planning.
  • Provide direction and supervision to Physical Therapist Assistants as appropriate.
  • Follow facility policies, professional standards, and applicable requirements.
  • Promote a positive therapy experience that respects each resident’s dignity and preferences.

Physical Therapist (PT) Gateway Post Acute Pay Range: $65.00–$70.00 per hour, based on experience Help Residents Move Forward—And Grow Your Career!
