Job summary

Gateway Post Acute in Porterville, CA is seeking a compassionate PRN Certified Occupational Therapy Assistant to join our rehabilitation team. This role offers flexible PRN scheduling and the chance to provide meaningful hands-on therapy.

You will deliver skilled OT treatments per the supervising therapist’s plan of care, help residents with activities of daily living, document care accurately, and collaborate with the interdisciplinary team on discharge planning and program development.

Qualifications

  • Associate degree from an accredited Occupational Therapy Assistant program required.
  • Current California Occupational Therapy Assistant license in good standing required.
  • Must maintain continuing education and licensure requirements.
  • Current CPR/BLS certification required when applicable.
  • Geriatric, post-acute care, or skilled nursing experience preferred.

Responsibilities

  • Provide skilled occupational therapy treatments according to the supervising OT’s plan of care.
  • Help residents improve activities of daily living (dressing, grooming, bathing, toileting, mobility).
  • Monitor responses to treatment and communicate progress or changes to the supervising OT.
  • Complete accurate, timely, and compliant clinical documentation.
  • Communicate with physicians, nurses, rehabilitation staff, residents, families, and team members.
  • Participate in care conferences, rehab meetings, and discharge planning as needed.
  • Educate residents and families on treatment goals, home programs, safety, and fall prevention.
  • Assist with adaptive equipment and durable medical devices as needed.
  • Provide training to nursing staff and in-services; follow infection control procedures.

Certified Occupational Therapy Assistant (COTA) – PRN Gateway Post Acute Pay Range: $42.00–$45.00 per hour, based on experience

Help Residents Regain Independence and Confidence Gateway Post Acute is seeking a compassionate and motivated PRN Certified Occupational Therapy Assistant to join our rehabilitation team. This is an excellent opportunity to provide meaningful, hands‑on therapy while enjoying the flexibility of a PRN schedule.

Essential Duties and Responsibilities
  • Provide skilled occupational therapy treatments according to the supervising Occupational Therapist’s plan of care.
  • Help residents improve their ability to complete activities of daily living, including dressing, grooming, bathing, toileting, feeding, and mobility.
  • Monitor residents’ responses to treatment and communicate progress, concerns, or changes to the supervising Occupational Therapist.
  • Complete accurate, timely, and compliant clinical documentation.
  • Communicate with physicians, nurses, rehabilitation staff, residents, families, and other members of the interdisciplinary team.
  • Participate in resident care conferences, rehabilitation meetings, and discharge planning as needed.
  • Educate residents, families, and caregivers regarding treatment goals, home exercise programs, safety techniques, fall prevention, and maintenance programs.
  • Recommend and assist with obtaining adaptive equipment, splints, assistive devices, elevated toilet seats, grab bars, and other durable medical equipment.
  • Assist nursing staff with training and supporting Restorative Nursing Assistants.
  • Provide employee in‑services and training programs as requested.
  • Follow infection prevention, safety, and equipment‑maintenance procedures.
  • Report damaged or malfunctioning equipment promptly.
  • Contribute to quality assurance, performance improvement, and rehabilitation program development.
  • Maintain resident privacy and the confidentiality of all resident and company information.
  • Perform additional duties as assigned.
Qualifications
  • Associate degree from an accredited Occupational Therapy Assistant program required.
  • Current California Occupational Therapy Assistant license in good standing required.
  • Must maintain all continuing education and licensure requirements.
  • Current CPR/BLS certification required when applicable.
  • Skilled nursing, post‑acute care, rehabilitation, long‑term‑care, or geriatric experience preferred.
  • New graduates are welcome to apply.
  • Experience with Net Health, PointClickCare, or other electronic documentation systems preferred.
  • Strong clinical, documentation, communication, and time‑management skills.
  • Ability to work independently while collaborating effectively with the supervising Occupational Therapist and interdisciplinary team.
  • Compassionate, professional, and resident‑centered approach to care.
  • Flexibility to provide coverage based on facility needs.
Physical Requirements

This position requires frequent standing, walking, reaching, pushing, pulling, and assisting residents with mobility and transfers. The employee may occasionally climb, balance, stoop, kneel, crouch, or crawl while providing care or operating equipment. The role may involve exerting more than 100 pounds of force occasionally, more than 50 pounds frequently, or more than 20 pounds regularly when safely assisting residents or moving equipment.
