Job summary

Gateway Post Acute in California is seeking a Part-Time Physical Therapist (PT) to evaluate and treat residents, develop individualized plans, and collaborate with an interdisciplinary team. The role emphasizes compassionate care, documentation accuracy, and adherence to facility policies.

The ideal candidate will hold a California PT license, CPR/BLS certification, and have strong assessment and communication skills, with preference for long-term care experience.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Evaluate residents' mobility, strength, balance, endurance, and functional abilities.
• Develop and implement individualized treatment plans based on assessments and resident goals.
• Provide therapeutic exercises, gait training, balance activities, and other interventions.
• Help residents improve transfers, mobility, and safe use of assistive devices.
• Monitor progress and adjust treatment plans to achieve meaningful outcomes.
• Educate residents, families, and caregivers on exercises and fall prevention.
• Maintain accurate evaluations, notes, progress reports, and discharge summaries.
• Collaborate with nursing, occupational, and speech therapy teammates.
• Participate in care planning and discharge planning.
• Provide direction and supervision to Physical Therapist Assistants.

Pay Range: $65.00-$70.00 per hour, based on experience

Key Responsibilities

• Evaluate residents' mobility, strength, balance, endurance, and functional abilities.
• Develop and implement individualized treatment plans based on assessments, physician orders, and resident goals.
• Provide therapeutic exercises, gait training, balance activities, and other appropriate interventions.
• Help residents improve transfers, mobility, and safe use of assistive devices.
• Monitor progress and adjust treatment plans to support meaningful outcomes.
• Educate residents, families, and caregivers on exercises, mobility techniques, and fall prevention.
• Maintain accurate and timely evaluations, treatment notes, progress reports, and discharge summaries.
• Collaborate with nursing, occupational therapy, speech therapy, and other interdisciplinary team members.
• Participate in care planning and discharge planning.
• Provide direction and supervision to Physical Therapist Assistants as appropriate.
• Follow facility policies, professional standards, and applicable requirements.
• Promote a positive therapy experience that respects each resident's dignity and preferences.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license in good standing.
• Current CPR/BLS certification.
• Previous skilled nursing or long-term care experience preferred.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication, organization, and interpersonal skills.
