Job summary

Valley View Post Acute is seeking a Staff Physical Therapist to evaluate residents, develop and update treatment plans, and support mobility and independence. You will supervise PT aides and collaborate with physicians and families to ensure coordinated care.

The role requires a PT license in California and proficiency with Net Health and Point Click Care, with a focus on infection control, safety, and accurate documentation. A dedicated team environment supports ongoing staff education.

Qualifications

• Bachelor's degree in Physical Therapy required; Masters/Doctorate preferred.
• Proficient in computer skills for Net Health and Point Click Care.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential per physician orders and facility policy.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Supervise PT assistants and aides in resident care and related activities.
• Communicate progress and goals with physicians, nursing staff, residents, and families.
• Participate in care conferences and rehabilitation meetings as needed.
• Complete timely and compliant documentation per facility, governmental, and payer requirements.
