Job summary

Valley-View-Post-Acute is seeking a Staff Physical Therapist to evaluate residents, develop and implement treatment plans, and supervise PT assistants. You will collaborate with physicians, nursing staff, and families to support rehabilitation goals and discharge planning.

The role requires licensure in the state of California, a PT degree (Master’s/Doctorate preferred), and strong documentation skills using Net Health and Point Click Care.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Licensed as a Physical Therapist in the state.
• Masters or Doctorate in Physical Therapy preferred.
• Proficient in computer skills and clinical documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate progress and discharge plans with physicians, nursing staff, residents and families.
• Participate in care conferences and rehabilitation meetings as needed.
• Maintain timely, accurate documentation in accordance with policies and payer requirements.
