Job summary

Valley View Post Acute El Monte SNF, LLC seeks a dedicated Physical Therapist Assistant to support the Staff Physical Therapist in resident care, perform assigned tasks, and implement treatment plans under supervision.

Responsibilities include documenting accurately, maintaining infection control and safety, assisting with training Restorative Aides, educating residents and families, and communicating progress to the care team to support timely discharge planning.

Qualifications

• Associate Degree from an accredited PTA program.
• Proficient in computer skills such as inputting data into Net Health and Point Click Care.
• Language Skills: ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Able to present information and respond to questions from managers, coworkers and families.
• Mathematical skills: fractions, percentages, ratios and proportions.
• Reasoning ability to solve practical problems.
• Ability to interpret written, oral, diagram, or schedule instructions.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Complete timely, accurate, and compliant documentation.
• Maintain a safe treatment environment following infection control and safety protocols.
• Participate in Resident Care conferences and rehabilitation meetings.
• Comply with California PTA laws and regulations where applicable.
• Assist nursing with training Restorative Aides.
• Communicate progress to physicians, nursing staff, and families.
• Educate residents and families on treatment goals and home programs.
