Job summary

Reliant Rehabilitation seeks a dedicated Physical Therapist to deliver high-quality rehabilitation services in skilled nursing facilities. You will evaluate patients, develop and adjust treatment plans, and coordinate discharge planning with care teams in a fast-paced environment.

The role requires active PT licensure, excellent communication, and proficiency with EMR systems. On-site position with opportunities for clinical leadership and continuing education support.

Qualifications

• Graduation from an APTA-accredited physical therapy program.
• Active state license as a physical therapist.
• Proficiency with electronic medical records and clinical documentation.

Responsibilities

• Provides physical therapy and related services to patients according to Reliant Rehabilitation guidelines.
• Maintains professional conduct and positive interactions with facilities and clients.
• Provides initial and ongoing treatment plans of care and discharge planning.
• Supervises and mentors PTAs and Rehab Techs as appropriate.
• Ensures compliance with state and federal regulations regarding physical therapy.
