Job summary

Apache-Junction-Health-Center is seeking a licensed Physical Therapist to evaluate and treat patients within 24 hours of referral. You will develop and implement care plans, supervise PT assistants, and document patient progress in Casamba and Point Click Care.

The role requires current AZ PT licensure, strong reading and communication skills, and the ability to instruct families for discharge planning. Eligible candidates will participate in care conferences and be part of a collaborative

Qualifications

• Ability to read and interpret technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to present information clearly to managers, coworkers, and families.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for an effective treatment plan from referring physician.
• Treat patients per the physician’s plan.
• Supervise PT assistants and therapy staff in patient care.
• Document evaluations and progress notes per policy and state rules.
• Instruct families and nursing staff on home exercise and discharge preparation.
• Participate in discharge planning and care conferences.
• Recommend durable medical equipment to facilitate patient independence.
