Job summary

PACS Group in Apache Junction, AZ, is seeking a licensed Physical Therapist to join our rehabilitation team. You will evaluate patients within 24 hours of referral, develop treatment plans, and treat patients per physician directives.

You will supervise PT assistants and students, document progress, and participate in discharge planning. Familiarity with Casamba and Point Click Care is preferred; licensure must be maintained.

Qualifications

• Licensed as a Physical Therapist in the state and maintain licensure.
• Proficient in Casamba and Point Click Care.
• Ability to read and interpret policy and procedure manuals.

Responsibilities

• Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Supervise Physical Therapy Assistants in direct patient care and patient related activities.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per PT Board State Practice Act and governmental and third party payer requirements.
• Record treatment changes per policy and procedures.
