Job summary

Apache-Junction-Health-Center is seeking a licensed Physical Therapist to evaluate and treat patients within 24 hours of referral, and to develop and secure approval for individualized treatment plans. The role includes supervising PT assistants and contributing to care conferences.

The incumbent will train families and nursing staff, participate in discharge planning, and ensure compliance with state practice acts and third-party payer requirements.

Qualifications

• Bachelor’s degree in Physical Therapy, prefer graduates of Masters or Doctorate Program in Physical Therapy.
• Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours, or 1 business day of physician referral.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Supervise Physical Therapy Assistants in direct patient care and related activities.
• Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
• Discharge planning and caregiver training for home programs.
