Job summary

Evolve Therapy Services in Florida seeks a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Pahokee.

You will provide PT services, evaluate residents, develop treatment plans, and ensure CMS-compliant documentation in NetHealth EMR, while collaborating with an interdisciplinary team.

Qualifications

• Minimum of BS Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill therapy services in CMS-compliant NetHealth EMR.
• Evaluate residents and develop short/long-term treatment goals with the team.
