Job summary

Evolve Therapy Services, LLC in Pahokee, FL is seeking a licensed Physical Therapist for an in-house Skilled Nursing Facility. You will provide patient-centered care, develop treatment plans, and collaborate with an interdisciplinary team to support residents.

The role requires a BS in Physical Therapy, an active Florida license, and ongoing continuing education. Full-time with a $5,000 sign-on bonus; part-time or PRN also available.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Commitment to maintaining current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct regular screenings to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant, medically necessary documentation in NetHealth EMR systems.
• Collaborate with the care team to ensure high-quality resident care.
