Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. You will plan, direct, and provide therapies, ensuring compliance with CMS expectations in NetHealth EMR, and collaborate with an interdisciplinary team to uphold high resident care standards.

Full-time opportunities are available with a $5,000 sign-on bonus; part-time or PRN roles may also be offered.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to develop treatment plans and goals.
• Conduct screenings to determine intervention needs and track progress.
• Develop and implement short/long term treatment plans with measurable goals.
• Communicate findings and recommendations to residents, families, and the interdisciplinary team.
