Qualifications

• BS in Physical Therapy from an accredited program
• Active Physical Therapy license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification
• Assume responsibility for ongoing continuing education to maintain licensure

Responsibilities

• Provide physical therapy services to residents per standards and procedures
• Document and bill services in NetHealth EMR
• Evaluate residents and develop treatment plans with short/long-term goals
• Conduct screenings to assess need for intervention or treatment
• Communicate findings and recommendations to residents, families, and interdisciplinary team
• Complete compliant CMS-ready documentation
