Evolve Therapy Services is seeking a Physical Therapist to provide in-house skilled nursing facility therapy services in Pahokee, FL. The role supports resident care across assessment, planning, and treatment with CMS-compliant documentation.

Eligible candidates hold a BS in Physical Therapy and an active state license, with ongoing CEU commitment. The position offers a $5,000 sign-on bonus for full-time, plus options for PT per diem or PRN.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention or treatment.
• Screen residents referred to PT to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete compliant and medically necessary CMS documentation in NetHealth EMR.
