Job summary

Evolve Therapy Services seeks a Physical Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. Full-time with a $5,000 sign-on bonus, plus part-time or PRN options, to deliver comprehensive PT services in partnership with the facility.

Responsibilities include evaluating residents, planning treatment, documenting care, and coordinating with families and the interdisciplinary team to ensure CMS-compliant, high-quality patient outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with standards and procedures.
• Adhere to documentation and billing procedures.
• Develop treatment plans with short- and long-term goals and communicate findings to residents and families.
• Complete CMS-compliant and medically necessary documentation in NetHealth EMR.
