Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Rochester, MN. This full-time or PRN position focuses on planning, implementing, and directing physical therapy services to ensure high-quality resident care.

Responsibilities include evaluating residents, creating treatment plans, documenting per CMS and company guidelines, and collaborating with the interdisciplinary team to achieve optimal outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill PT services per company policy.
• Evaluate residents and develop short/long-term treatment goals.
• Conduct screenings to determine need for intervention or treatment.
• Communicate findings and plans with residents, families, and the care team.
• Complete compliant CMS-documented records in NetHealth EMR.
