Job summary

UChicago Medicine in Harvey, IL is seeking a Level I Physical Therapist to provide comprehensive assessment, treatment, and rehabilitation under supervision. This full-time role emphasizes patient-centered care, documentation, and collaboration with the therapy team to support diverse populations.

The incumbent will supervise PTA/ Rehab Aide, participate in education of patients and interns, and contribute to discharge planning and care coordination while adhering to all policies and standards.

Qualifications

• Requires graduation from an approved physical therapy school.
• Requires licensure to practice physical therapy in Illinois (or eligibility).
• CPR/BLS certification included or required.
• Experience with a range of ages from neonatal to geriatric is desirable.

Responsibilities

• Conducts evaluations using tests and measurements to determine PT needs and plans.
• Collaborates with interdisciplinary team to establish patient disposition.
• Supervises PTA/ Rehab Aide and interns; orients and mentors staff.
• Performs and documents PT interventions and procedures.
• Coordinates care and ensures effective communication for discharge planning.
