Job summary
UChicago Medicine in Hyde Park (Chicago, IL) seeks a Physical Therapist II to deliver comprehensive evaluation, treatment, and rehabilitation across adult and geriatric populations. The role includes mentoring students and contributing to performance improvement efforts.
Eligible for a $1500 sign-on bonus, you will collaborate with the multidisciplinary team, document care thoroughly, and participate in evidence-based practice development within the Therapy Services department.
Qualifications
- Graduation from an approved physical therapy program.
- Minimum of 2 years’ clinical physical therapy experience.
- APTA CI Credentialing Course completion requested.
- Current Illinois physical therapy license or eligibility to practice.
- CPR/BLS certification is required.
Responsibilities
- Perform comprehensive patient evaluations to determine PT needs and plans.
- Collaborate with interdisciplinary team to set disposition and goals.
- Provide age-appropriate patient care for neonatal to geriatric populations.
- Document evaluations, progress notes, and procedures accurately and timely.
- Mentor PT/PTA students and junior staff; participate in quality and performance improvement initiatives.
