Job summary

University of Chicago Medicine is seeking a Level II Physical Therapist for inpatient care in Hyde Park, Chicago, IL. This full-time Days role offers a $1500 sign-on and focuses on comprehensive assessments, rehabilitation, and collaboration with an interdisciplinary team.

The ideal candidate has 2+ years of PT experience, Illinois license, and APTA CI credentialing, with a drive for quality improvement and mentorship of students and staff.

Qualifications

• Requires graduation from an approved physical therapy school.
• Minimum of 2 years’ clinical physical therapy experience.
• Declaration of intent to develop advanced, clinical knowledge and skills in a focused area of clinical practice.
• Demonstrated sustained active engagement in performance and quality improvement initiatives and/or program development.
• Completion of APTA CI Credentialing Course
• Requires current physical therapy license in the State of Illinois (or eligibility to practice) and CPR/BLS certification.

Responsibilities

• Conducts comprehensive evaluations using appropriate tests and measurements to determine physical therapy problems, functional levels, and recommended treatment plans.
• Collaborates with the interdisciplinary team to establish patient disposition.
• Demonstrates knowledge of and applies patient care protocols for neonatal, pediatric, adult and geriatric populations.
• Adheres to departmental policies, procedures and regulatory requirements.
• Actively participates or leads performance and quality improvement activities.
• Develops physical therapy interventions in collaboration with the patient/family, referring practitioner and treatment team goals.
• Competently performs appropriate physical therapy techniques and procedures.
• Ensures continuity of care through timely and effective written and verbal communication with team members and other treating disciplines.
• Attends rounds, conferences, and clinics as assigned, providing patient updates and discharge recommendations.
• Identifies and recommends necessary patient care equipment prior to discharge and assists with procurement as appropriate.
• Accurately documents evaluations, progress notes, and all procedures performed within established timeframes.
• Completes compliant, timely, and accurate patient billing at the end of each day.
• Instructs patients and families in home exercise programs.
• Provides education on therapy diagnosis, prognosis, and treatment plans.
• Seeks an advanced level of practice or specialty.
• Participates in departmental and/or organizational committees as assigned.
• Assumes lead responsibility in the absence of the supervisor or manager as assigned.
