Advocate Health in Charlotte, NC seeks a licensed Physical Therapist to deliver comprehensive therapy services in a fast-paced clinical environment. You will perform patient evaluations, develop individualized care plans, and document progress for all episodes of care.

The role requires supervising PT aides and assistants, coordinating discharge planning, and communicating effectively with physicians and care teams to ensure seamless treatment continuity.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• Current license to practice Physical Therapy in the applicable state.
• BLS certification is required by policy.

Responsabilités

• Provides direct therapy services to patients seven days a week, including holidays.
• Evaluates patients and develops a patient care plan with modalities, procedures, and education.
• Gathers and interprets data from medical records and family to inform care.
• Documents evaluations, tests, progress notes, discharge plans and charges as needed.
• Communicates progress to physicians and other clinical staff; coordinates schedules and discharge planning.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
