Job summary

Advocate Health in Charlotte, NC is seeking a full-time Physical Therapist to evaluate patients, design and implement comprehensive care plans, and supervise PTA’s and Rehab Tech/Aide. The role involves coordination with physicians, family, and other staff to ensure timely discharge planning and optimal outcomes.

Requirements include a Bachelor’s degree in Physical Therapy, an applicable state license, and BLS certification.

Qualifications

• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluates patients and establishes appropriate patient care plan.
• Gathers and interprets data from medical records and discussions with patient/family.
• Establishes functional goals and provides evidence-based interventions to meet them.
• Documents evaluations, tests, progress notes, discharge notes and charges as needed.
• Communicates evaluation and progress to physicians and other staff; coordinates schedules and discharge planning.
• Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
