Advocate Health is seeking a licensed Physical Therapist to provide home-based outpatient rehabilitation in the Northern Charlotte area. You will deliver individualized, evidence-based therapy in patients' homes, promoting mobility and independence while coordinating care with family and physicians.

The role emphasizes autonomy, relationship-based care, and expanding access to rehabilitation services in the community. Requires a PT license and a Bachelor's degree in Physical Therapy.

Qualifications

• Bachelor's degree in Physical Therapy and state license.
• BLS certification required.
• Excellent communication and collaboration with care team.

Responsibilities

• Deliver high-quality, one-on-one outpatient physical therapy services in patients’ homes.
• Develop individualized treatment plans and collaborate with physicians, caregivers, and interdisciplinary team members.
• Utilize technology and flexible scheduling to manage a caseload across a defined geographic area.
• Provide supervision of PTAs and Rehab Tech/Aide as required for licensure.
• Communicate evaluations and progress to physicians and other clinical staff; coordinate scheduling and discharge planning.
