Job summary

Talin Home Health is seeking a dedicated Physical Therapist to provide in-home evaluations and treatment to patients across Virginia. You will design personalized care plans and collaborate with a care team to support recovery and independence.

You will employ exercises, manual therapy, and modalities, educate patients and families on home programs, and document progress accurately, helping patients regain function and quality of life.

Qualifications

• DPT from an accredited program.
• Experience in orthopedics or home health care preferred.
• Strong knowledge of medical terminology and anatomy.
• Ability to coordinate discharge planning.
• Excellent interpersonal skills.

Responsibilities

• Conduct thorough evaluations of patients’ physical conditions and functional abilities.
• Develop individualized care plans that address the specific needs of each patient.
• Implement treatment programs utilizing exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Educate patients and their families on the rehabilitation process, home exercises, and injury prevention strategies.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning for inpatient cases.
• Maintain accurate documentation of patient evaluations, treatments, and progress in accordance with clinic policies.
