Job summary

Talin Home Health is seeking a dedicated Physical Therapist to evaluate and treat patients in their homes, improving function and quality of life.

You will develop individualized plans, implement physical therapy techniques, and coordinate with healthcare teams to ensure comprehensive care and smooth discharge planning where needed.

Qualifications

• Licensed/degree in Physical Therapy from an accredited institution.
• Experience in orthopedics or home community care preferred.
• Strong knowledge of medical terminology and anatomy.
• Familiarity with discharge planning processes is a plus.
• Excellent collaboration with healthcare teams.

Responsibilities

• Conduct thorough evaluations of patients’ physical conditions and functional abilities.
• Develop individualized care plans addressing each patient’s needs.
• Implement treatment programs using exercises, manual therapy, and modalities.
• Monitor progress and adjust plans for optimal outcomes.
• Educate patients and families on rehabilitation and home exercises.

Responsibilities

• Conduct thorough evaluations of patients’ physical conditions and functional abilities.
• Develop individualized care plans that address the specific needs of each patient.
• Implement treatment programs utilizing various therapeutic techniques, including exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.

• Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning for inpatient cases.

Experience

• A degree in Physical Therapy from an accredited institution is required.
