Interim HealthCare Inc. is seeking a Home Health Physical Therapist (PT) to provide one-on-one therapy in patients’ homes across Christiansburg and surrounding areas. The role offers full-time or PRN scheduling, with pay per visit and mileage reimbursement.

The PT will assess patients, create individualized plans, and support recovery and mobility in the home setting while collaborating with a broad clinical team.

Qualificações

• Graduated from an accredited Physical Therapist program, VA license required.
• Minimum 2 years PT experience, home health preferred.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Ability to travel throughout the service area.
• Strong clinical judgment and compassionate demeanor.

Responsabilidades

• Provide home-based physical therapy to patients recovering from illness or injury.
• Conduct patient assessments and develop individualized treatment plans.
• Monitor progress and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on therapy goals, exercises, and self-care techniques.
• Coach family members on safely assisting with exercises and patient care.
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, OT, and SLP.
• Maintain accurate and timely documentation of patient progress.

Schedule: Full-Time / PRN (Flexible Scheduling Available)

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist - PT to provide personalized, one-on-one therapy in a home setting throughout Christiansburg and the surrounding service area. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist - PT, you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.

Responsibilities of Home Health Physical Therapist - PT:

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
