Job summary

Interim HealthCare Inc. seeks a Home Health Physical Therapist (PT) to provide one-on-one, in-home therapy throughout Roanoke and the surrounding service area.

This part-time/PRN role offers flexible scheduling and the opportunity to help patients regain strength and mobility in their own homes. You will develop individualized treatment plans, assess progress, educate patients and families, and collaborate with an interdisciplinary team while traveling to patient homes and maintaining detailed

Qualifications

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification required
• Knowledge of state and federal home health regulations
• Strong clinical judgment, interpersonal skills, and compassion
• Ability to work independently in a home setting
• Willingness to travel throughout the service area

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on therapy goals, exercises, and self-care techniques
• Coach family members on safely assisting with exercises and patient care
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, OT, and SLP
• Maintain accurate and timely documentation of patient progress

Skills

Graduate of an accredited PT program

Active PT license Virginia

2+ years home health PT experience

CPR certification

Knowledge of home health regulations

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview:

We are currently seeking a compassionate and skilled Home Health Physical Therapist - PT to provide personalized, one-on-one therapy in a home setting throughout Roanoke and the surrounding service area. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist - PT, you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.

Responsibilities of Home Health Physical Therapist - PT:
