Qualifications

• Graduate of an accredited Physical Therapist program.
• Active PT license in the state of Virginia.
• Minimum of 2 years of physical therapy experience (home health preferred).
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Strong clinical judgment, interpersonal, and critical thinking skills.
• Compassionate, dependable, and patient-focused.
• Ability to work independently in a home setting.
• Willingness to travel throughout the service area.

Responsibilities

• Provide home-based physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on therapy goals, exercises, and self-care techniques
• Coach family members on safely assisting with exercises and patient care
• Collaborate with interdisciplinary team including RN, LPN, CNA, HHA, OT, and SLP
• Maintain accurate and timely documentation of patient progress

Job Title: Home Health Physical Therapist - PT

Location: Dublin / Radford, VA & Surrounding Areas (In-Home Patient Care)

Schedule: Full-Time / Part-Time / PRN (Flexible Scheduling Available)

Pay: $45-$100 per visit depending on visit type + mileage reimbursement

Job Overview

We are currently seeking a compassionate and skilled Home Health Physical Therapist - PT to provide personalized, one-on-one therapy in a home setting throughout Dublin, Radford, and the surrounding coverage areas. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist - PT, you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.
