Job summary

Talin Home Health Care LLC is seeking a dedicated Occupational Therapist to provide in-home therapy, focusing on improving functional abilities for patients with disabilities, injuries, and aging-related needs. You will assess, plan, and implement personalized treatment to enhance daily living activities.

Qualifications

• Requires OT degree from an accredited program.
• Valid state licensure as an Occupational Therapist.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management and working with individuals with disabilities preferred.
• Familiarity with early intervention strategies is a plus.
• Excellent communication skills with patients, families, and teams.
• Ability to develop effective care plans addressing individual patient needs.

Responsibilities

• Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
• Develop and implement individualized care plans tailored to each patient's goals and capabilities.

The ideal candidate will be responsible for providing high-quality patient care in a patient's homes, focusing on improving the functional abilities of individuals with disabilities, injuries, recovery and aging. This role involves developing personalized care plans, conducting assessments, and implementing therapeutic interventions to enhance patients' quality of life.
