Role Overview
The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions within an interdisciplinary team.
Key Responsibilities:
- Perform patient assessments and develop individualized treatment plans.
- Deliver therapeutic interventions to improve patient mobility and function.
- Contribute to program development, quality improvement, and productivity goals.
- Supervise Physical Therapy Assistants and Home Health Aids.
Qualifications & Skills
- Licensed or qualified to perform physical therapy with ethical standards.
- Strong assessment, treatment, and interdisciplinary collaboration skills.
- Accountability for quality and program development.
Physical & Visual/Hearing Requirements
- Ability to lift, push, or pull loads up to 50 pounds.
- Mobility to frequently change positions and assist patients.
- Sufficient strength and stamina for physical activities and location changes.
- Visual acuity with corrective lenses and hearing clarity with assistance.
- Exposure to infectious diseases and potential handling of emergency situations.
- May be required to wear protective equipment and handle chemicals.
Work Conditions
- Activities include sitting, standing, walking, bending, squatting, climbing, kneeling, and twisting.
- Frequent walking and standing; occasional climbing and kneeling.
- Occasional activity changes and exposure to various environmental conditions.
