Job summary

DayToDay Health is seeking a Home Health Physical Therapist to provide patient-centered care in clients' homes, promoting recovery, mobility, and independence. The role emphasizes autonomy in field-based work and collaboration with a multidisciplinary care team.

You will evaluate functional status, develop personalized plans, deliver hands-on therapy, and educate patients and families on safe exercises and treatment goals. Strong communication and a valid state license are required.

Qualifications

• Graduate of an accredited PT program (DPT or Master’s).
• 1–2 years of home health experience preferred.
• Strong clinical assessment, critical thinking, and communication skills.
• CPR certification and valid driver’s license with reliable transportation.
