Job summary

Consumer Directed Services Inc. seeks a licensed Physical Therapist to provide primary rehabilitative care to home health patients across multiple counties, coordinating interdisciplinary services and overseeing therapy plans from admission to discharge.

You will supervise PTAs and aides, ensure regulatory documentation, and participate in quality improvement activities while promoting patient independence and positive outcomes.

Qualifications

• A graduate of an accredited physical therapy program with a valid license, BLS certification, and a valid driver’s license with private vehicle insurance.
• At least one year of full-time PT experience, preferably in home health.
• Must have strong assessment, communication, organizational, and problem-solving skills, with proficiency in home health operations and familiarity with OASIS preferred.

• Coordinating with team members to ensure goals are met and revising plans as needed.
• Providing patient and caregiver education to promote independence and positive outcomes.
