This position requires hands‑on patient care in their residential settings, focusing on functional mobility, reducing pain, and preventing disability. The chosen candidate will be expected to travel to various home locations to provide personalized therapy and care tailored to individual patient needs.

Responsibilities

• Conduct assessments of patients’ physical conditions based on their medical history and physician referrals.
• Develop and implement individualized care plans that promote health and treat conditions.
• Provide direct patient care aimed at improving mobility and alleviating pain.
• Instruct patients and their families on proper home‑care strategies and exercise routines.
• Maintain accurate and detailed patient records, including conditions, treatments, and progress.
• Regularly evaluate and update patient care plans in collaboration with the healthcare team.
• Engage in patient and family education about lifestyle choices, proper body mechanics, and the use of assistive devices.
• Apply therapeutic treatments using physical agents and techniques, which may include massage, stimulation, therapeutic exercises, and hydrotherapy.
• Travel to patients’ homes to deliver physical therapy sessions and follow‑up on their care.
• Coordinate with other healthcare professionals to maximize patient recovery and progress.

Qualifications
