Job summary

Northern Counties Health Care, Inc. seeks a Home Health and Hospice Physical Therapist to assess, plan, and deliver physical therapy for patients in home care and hospice settings.

You’ll participate with an interdisciplinary team to promote mobility and independence. Responsibilities include completing OASIS assessments, developing a comprehensive plan of care, and educating patients and caregivers on therapy goals and use of devices.

Qualifications

  • Must have a bachelor’s degree in physical therapy from an accredited program.
  • Licensed to practice as a physical therapist in the state.
  • Two years of relevant physical therapy experience, community/home health preferred.
  • Current CPR certification.
  • Good observation, communication, problem-solving and basic math skills.

Responsibilities

  • Provide PT services to home care and hospice patients per physician’s plan of care.
  • Assess and evaluate therapeutic status and develop the total plan of care.
  • Complete OASIS assessments as indicated.
  • Direct PT treatment and instruct patients and families in use of therapeutic appliances.
  • Identify needs for other home health services and refer as appropriate.
  • Prepare clinical and progress summaries and reports.

Job Summary:

The Home Health and Hospice Physical Therapist is responsible for assessing, developing a plan of care and providing physical therapy services to home care and hospice patients of all ages. The physical therapist is an essential part of an interdisciplinary team.

Supervisory Responsibilities:

This position has no supervisory responsibilities.

Essential Job Functions/Responsibilities:

  • Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:
  • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
  • Assessing for muscle strength, mobility, gait, ROM—potential for rehab.
  • Developing comprehensive patient plan of care.
  • Completing OASIS assessments as indicated.
  • Directing physical therapy treatment.
  • Instructing patients and families/caregivers in the use and care of therapeutic appliances.
  • Determining priority needs for physical therapy.
  • Reporting to physician patient’s reaction to treatment or changes in condition.
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
  • Identifies patient and family/caregiver needs for other home health services and refers as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals.
  • Provides physical therapy consultation to home families/caregivers when indicated.
  • Provides in-service education programs for nursing organization personnel as needed.
  • Participates in peer consultation process.
  • Supervises physical therapy assistants according to organization policy and state regulations.
  • When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

Position Qualifications:

  • Possesses a minimum degree from a baccalaureate program in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapist within the state, if applicable.
  • Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
  • Possesses and maintains current CPR certification.
  • Complies with acceptable professional standards and practice.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist.
  • Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
  • May occasionally be in a position to lift and/or carry an item weighing up to 50 pounds.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
