Job summary

LiveWell Homecare Agency in Texas is seeking a dedicated Home Health Physical Therapist to provide comprehensive PT services in an interdisciplinary team. You will assess patients, develop individualized treatment plans, and implement therapeutic interventions in home health settings.

You will supervise clinical staff, contribute to program development and quality standards, and adapt care to infectious disease control and emergency situations, with emphasis on safety and patient mobility.

Role: The Home Health Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and therapeutic interventions, in an interdisciplinary environment. They are accountable for program development, quality improvement, and productivity enhancement, collaborating flexibly with team members.

Supervises: Positions such as Physical Therapy Assistant and Home Health Aides.
