Job summary

Billet Health is seeking a compassionate Home Health Physical Therapist (PT) for the West Valley, AZ area. This per diem role offers autonomy to provide one-on-one patient care in patients' homes.

You will evaluate and treat patients at their homes to improve mobility and independence, with a flexible schedule and per diem compensation.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Arizona Physical Therapist license.
• Valid driver's license and reliable transportation.
• Current TB documentation and CPR/BLS certification.
• Home Health experience preferred.

Responsibilities

• Perform comprehensive patient evaluations and assessments.
• Develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in patients' homes.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on exercises, mobility, and safety.
• Collaborate with physicians and interdisciplinary team members.
• Complete accurate and timely documentation.

Billet Health is looking for a compassionate and dedicated Physical Therapist (PT) to join our Home Health team in the West Valley. This per diem opportunity offers flexibility and autonomy while allowing you to provide one-on-one care that truly makes a difference.

As a Home Health Physical Therapist, you will evaluate and treat patients in their homes, helping them regain mobility, improve function, and maintain independence.

Responsibilities

• Perform comprehensive patient evaluations and assessments
• Develop individualized treatment plans based on physician orders
• Provide skilled physical therapy services in patients' homes
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, mobility, and safety
• Collaborate with physicians and interdisciplinary team members
• Complete accurate and timely documentation

• CPR or BLS certification
• Current TB documentation (skin test within one year or chest X-ray within five years)
