Interim Healthcare of the Twin Cities is seeking a Home Care Physical Therapist to provide one-on-one in-home care across a defined region, primarily in the southwest Twin Cities metro. The role offers independence and autonomy with weekend rotation and a strong emphasis on patient outcomes.

You will travel within the service area, maintain detailed documentation, and collaborate with a care team to support patient goals and safe discharge planning.

Qualifications

• Licensed Minnesota Physical Therapist.
• 2+ years of physical therapy experience preferred.
• Active CPR/BLS certification.

Responsibilities

• Assist physicians in evaluating patient function and plan of care.
• Develop and adjust the care plan with patient and caregivers.
• Regularly assess patient status and modify treatment as needed.
• Advise families to promote progress toward goals and discharge planning.
• Coordinate with the interdisciplinary team to ensure continuity of care.
• Complete timely and accurate documentation of care provided.

Hours: Full-time - Days, Monday - Friday | There will also be a weekend rotation. Dependent upon staffing levels, work one weekend approximately every 6 weeks.

Territory:

The Physical Therapist will have a general territory within our service area where they will provide one on one in-home care to the patients. The office is in Roseville, MN and the general territory for this position would primarily be in the southwest section of the Twin Cities metro area.

A Few Must-haves for This Position:

• Currently licensed as a Physical Therapist in the state of Minnesota.
• In the last five (5) years, two (2) years of experience as a Physical Therapist that is relevant to the patient population assigned preferred.
• Able to travel within geographic area serviced by office. Valid drivers license, reliable transportation,and proof of auto insurance is required.
• CPR/BLS certification

Some functions of what you will do as a Physical Therapist:

• Assists the Physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs as well as in developing the resulting plan of care/treatment including appropriate long and short-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Regularly assesses changes in the patient’s status since the last visit/treatment day, and determines whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the Physician as indicated.
• Advises and consults with the family and/or other caregivers to promote the patient progress toward mutually agreed upon goals and planning for discharge.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote the coordination of patient care and planning for discharge.
• Completes and submits timely, legible, accurate, relevant and sufficient documentation
