Job summary

Interim Healthcare of the Twin Cities seeks a licensed Physical Therapist to provide one-on-one in-home care within Roseville, MN and surrounding areas. The role emphasizes independence, scheduling flexibility, and comprehensive patient assessments with a focus on functional goals.

The position requires Minnesota licensure, travel within the service area, and CPR/BLS certification. Weekend rotations and a competitive benefits package are included to support quality patient outcomes.

Qualifications

• Licensed Physical Therapist in Minnesota.
• 2+ years of PT experience in relevant patient population preferred.
• Must be able to travel within service area with valid driver’s license and transportation.
• CPR/BLS certification.

Responsibilities

• Assist physicians in evaluating patient function and equipment needs and develop plan of care and treatment with appropriate goals and intensity.
• Develop and evaluate plan of care in partnership with patient and caregiver(s).
• Regularly assess changes in patient status and modify services as needed with physician orders when indicated.
• Advise and consult with family/caregivers to progress toward goals and discharge planning.
• Coordinate with interdisciplinary team to promote discharge planning and patient care continuity.
• Complete and submit accurate, timely documentation.

Hours: Full-time - Days, Monday - Friday | There will also be a weekend rotation. Dependent upon staffing levels, work one weekend approximately every 6 weeks.

Territory:

The Physical Therapist will have a general territory within our service area where they will provide one on one in-home care to the patients. The office is in Roseville, MN and the general territory for this position would primarily be in the southwest section of the Twin Cities metro area.

A Few Must-haves for This Position:

• Currently licensed as a Physical Therapist in the state of Minnesota.
• In the last five (5) years, two (2) years of experience as a Physical Therapist that is relevant to the patient population assigned preferred.
• Able to travel within geographic area serviced by office. Valid drivers license, reliable transportation,and proof of auto insurance is required.
• CPR/BLS certification

Some functions of what you will do as a Physical Therapist:

• Assists the Physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs as well as in developing the resulting plan of care/treatment including appropriate long and short-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Regularly assesses changes in the patient’s status since the last visit/treatment day, and determines whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the Physician as indicated.
• Advises and consults with the family and/or other caregivers to promote the patient progress toward mutually agreed upon goals and planning for discharge.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote the coordination of patient care and planning for discharge.
• Completes and submits timely, legible, accurate, relevant and sufficient documentation
