Interim HealthCare of the Twin Cities is seeking a Licensed Physical Therapist Assistant to provide in-home patient care within the Roseville, MN area. The role offers independence with a Monday–Friday 8:00am–5:00pm schedule and a field-based territory in the southeast metro.

Responsibilities include delivering therapy under a PT, conducting assessments, documenting progress, and educating patients and families to support goal achievement. Travel within the service area is required.

Qualificações

• License as Physical Therapist Assistant in MN, active and in good standing.
• Minimum 2 years of PTA experience.
• Valid driver’s license with reliable transportation.
• CPR/BLS certification required.

Responsabilidades

• Provide PTA services under supervision of a licensed PT.
• Participate in interdisciplinary patient assessment.
• Deliver services as ordered by physician and plan of care.
• Complete timely, legible documentation of treatments and patient progress.
• Counsel patient and family to promote progress toward goals.

The Licensed Home Care Physical Therapist Assistant (PTA) will enjoy independence and flexibility in the field to care for patients in their home environment.

Hours

Full-time | Monday - Friday | 8:00am - 5:00pm

Territory

This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southeast metro.

A few must-haves for this position

• Currently licensedas a Physical Therapist Assistant in the State of Minnesota. License is active and in good standing.
• Two (2) years of experience as a PTA.
• Able to travel within geographic area serviced by the office.Valid driver'slicense, reliable transportation, and proof of auto insurance.
• CPR/BLS Certification

Here are some functions of what you would do as a Licensed Physical Therapist Assistant

• Provide services for treating impairments, functional limitations, disabilities or changes in physical function under the direction and supervision of a qualified physical therapist.
• Participates in ongoing interdisciplinary assessment of the patient.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Completes and submits timely, legible, accurate, relevant and sufficient documentation addressing the treatment and interventions provided, the patient’s response to the treatment and interventions, and the patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and payer requirements.
• Counsels and educates the patient and the family, and other caregivers to promote patient progress toward mutually established goals and in meeting healthcare related needs.
