Job summary

CoxHealth is seeking a licensed Physical Therapist for in-home home health services in Springfield. The role delivers PT evaluations and treatments to relieve pain, restore function, and maintain maximum physical performance in patients at their homes.

Requires PT license and ability to travel to patient locations. Typically 40-hour weeks with day shifts and a focus on home-based care for diverse patient populations.

Qualifications

• Graduate of approved Physical Therapy program and licensure.
• Two years experience as Physical Therapist and satisfactory proficiency exam per 54 FR 33354.

Responsibilities

• Provides in-home evaluation and application of Physical Therapy services to relieve pain and restore function.
• Develops and implements individualized treatment plans.
• Coordinates with patients and families, documents progress and outcomes.

Job Description

Provides in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance of patient.

Education

• Required: Graduate of approved program in Physical Therapy as referenced in Conditions of Participation 54 FR 33354 (@484.4), or Two years experience as Physical Therapist and satisfactory grade on proficiency exam conducted, approved, or sponsored by U.S. Public Health Service. (N/A if licensed or sought qualification as PT after December 31, 1977).

Experience

• Preferred: 1-year Physical Therapy experience.

Skills

• Ability to provide reliable transportation as required by nature of position.

Licensure/Certification/Registration

• Required: Current Physical Therapist license or registration as required by state
• Required: Active Driver’s License
