Job summary

CoxHealth is seeking a dedicated Physical Therapist to provide in-home evaluation and therapy services through CoxHealth at Home. The role focuses on relieving pain, restoring function, and maintaining optimal mobility within patients’ homes in the Springfield area.

Requirements include graduation from an approved Physical Therapy program and an active PT license, with a preference for 1 year of home health experience and a valid driver's license for reliable transportation to patient homes.

Qualifications

• Graduate of an approved Physical Therapy program.
• Preferred: 1 year Physical Therapy experience.
• Current Physical Therapist license or registration as required by state.
• Active Driver's License.

Responsibilities

• Provide in-home evaluation, direction, and application of Physical Therapy services to relieve pain, develop or restore function, and/or maintain maximum physical performance.
• Coordinate care with patients and families in home health setting.
• Document progress and adjust treatment plans as needed.
