Job summary

Total Care Therapy in Granger, IN is seeking a compassionate Physical Therapist to join our clinician-led team. You will evaluate residents, develop individualized treatment plans, and deliver skilled therapies to improve mobility and independence for older adults in assisted living and independent living communities.

Qualifications

• Active Physical Therapist license in the state.
• CPR/BLS certification required.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Skills
