Job summary

Total Care Therapy is hiring a compassionate Physical Therapist for our Grand Rapids, MI locations. This full-time role focuses on evaluating and treating older adults to improve mobility, reduce pain, and restore independence across assisted living and related communities.

You will work within a therapist-owned, growth-focused culture, collaborating with an interdisciplinary team and mentoring PTAs while delivering evidence-based care in a supportive environment.

Qualifications

• Experience working with older adults in clinical settings preferred.
• Strong assessment and gait training skills.
• Comfort with interdisciplinary collaboration and documentation.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise and gait training.
• Reassess progress and modify plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely documentation in the EMR.
• Educate residents and staff on treatment plans and home exercise programs.
• Collaborate with the interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload with high-quality therapy.
