Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our therapist-owned team in Sterling Heights, MI. You will evaluate residents, develop individualized treatment plans, and deliver therapeutic exercises to improve mobility and independence in assisted living and independent living communities.

Qualifications

• PT license required.
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise PTAs in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
