Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join our Dayton team. You will evaluate, develop, and implement individualized treatment plans to improve residents’ mobility, restore function, reduce pain, and maintain independence in assisted living and independent living settings.

Qualifications

• Current State Physical Therapist (PT) license is required.
• Current CPR/BLS certification is required.
• Valid driver’s license with reliable transportation and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on treatment plans and safety techniques.
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering high-quality therapy services.
